Date: 2026-04-27 Scope: Review Opus P9 Tier 2 Remediation Design v0.2 after 5 governance patches.
Verdict
PASS. v0.2 correctly patched the governance risks identified in v0.1. It remains design-only and does not authorize production mutation.
Key checks
- Đ24 facet remediation now follows governed flow: DOT detect → issue/APR → approval → DOT/API create. No APR = no facet creation.
- entity_code contract is scoped to D38 domain only and no longer claims to be global Đ24 standard.
- Regex covers ROOT, section, paragraph, and future deep nesting forms.
- system_issues adapter is blocked until read-only verification of fn_log_issue and actual column mapping.
- dot-dot-register v2 is split into design/dry-run/verify before any dot_tools write.

- NT4: PASS. Alias/config mapping is governance/config, not hardcoded production behavior.
