Three Declarations

1. Permanent: repair the reusable policy/identity/permit/ledger/runtime gates, not one batch.
2. Mistake-proof: prevent false done, over-scope permits, collisions, and absent-runtime false PASS.
3. Automatic: metadata-driven bounded apply, failure/resume, and rerun proof without manual SQL births.
