FIX7 Readiness Denominator Specification

Denominator is every row in the single ACTIVE READINESS_GATE manifest. Seed exact set has 14 gates: OBJECT_AUTHORITY_IMMUTABLE, READINESS_MANIFEST_EXACT, SIGNOFF_AUTHENTIC, CAPABILITY_BEHAVIORAL, DEPENDENCY_TRUTH, NO_BYPASS_ALL_BLOCKED, CANONICAL_HASH_SHA256, PLANNER_NOT_CLONE, SCALE_SAFE, GATEWAY_PASS, FREEZE_PASS, PERMIT_POLICY_PASS, TIER_POLICY_PASS, WRITER_FAIL_CLOSED. Each has fixed adapter name/rule set/freshness in the full spec.

Ready requires exact expected=actual gate and fact sets in both EXCEPT directions, correct count/hash/epoch/source/evidence, no NULL/missing/extra/unknown/stale, every typed rule true, Directus no-write, cutover complete and post-activation verifier complete. bool_and alone forbidden. Delete failing result, fake/extra result, NULL, stale/wrong hash, Directus mutation, reduced manifest without exact critical quorum, or old epoch all fail.

BYPASS_VECTOR manifest seed exact set has 14 named vectors covering Directus owner/DML/DDL/execute, PUBLIC execute, runtime target DML, unknown dependency/dynamic SQL, manifest exactness, spoof/self-attest routes, quorum, epoch and legacy writer.
