P-02 - H04 Scope Hash Key List

Codex chose a stronger definition than T1's four-key minimum. H04_SCOPE_V1 uses domain incomex.qt001.signoff-scope.v1 and exact ordered keys:

1. activation_id
2. target_manifest_id
3. plan_content_hash
4. quorum_profile_id
5. required_principal_class_id
6. slot_ordinal
7. action_id
8. control_epoch

Every value comes from the same signoff_binding row and is required. Canonical PG16 JSONB plus SHA-256 is used. This prevents slot-scope replay across activation, target, plan, or epoch while binding the same definition consumed by H04, slot uniqueness, and same-human separation.

The domain constant versions the wire protocol; it does not encode mutable business policy. This defines the existing signoff_binding.scope_hash sub-payload and does not add an eighth top-level hash contract; H01..H07 remain the exact contract set.
