FIX7 CP-06 Hash Micro-Patch From T1 Proposals

Codex independently accepted and applied T1 P-01..P-04. P-01 pins evidence-content dereference; P-02 defines complete H04_SCOPE_V1; P-03 pins four runtime aggregate membership/order contracts; P-04 closes extra-index disposition with no runtime exception: every extra index fails.

This patch changes no production object and introduces no authority surface, readiness gate, hash contract, runtime table, or Directus authority. It does not grant final approval. T1 must perform one final short-review before Codex may promote the package to official design.
