BIRTH_GATEWAY_DESIGN_INDEX.md patched to revision 16 with a 2.6A-FIX2 banner recording: the second Codex rejection answered; parity BLOCK-only; fail-closed rule engine (governed registry_v2); content-addressed plan fingerprint over full scope + all control inputs; exact signoff; readiness v5 (no literals, BLOCKED); writer in-body governance lockdown; 13/13 blockers fixed/routed; birth-neutral; apply and permit still BLOCKED; fresh independent Codex re-audit required before 2.6B. Next macro recorded: BIRTH_STAGE2_6B_QT001_PERMIT_RUN_KEYSET_RESUME_AND_AUTHORITY_REVOKE.

The prior 2.6A and 2.6A-FIX entries remain; 2.6A-FIX status is superseded by FIX2. Stage 2.6B is gated on independent re-audit + permit/run/keyset/resume layer + PUBLIC EXECUTE revoke / role separation + perf gates.
