Last 24h H11a created 8,000 rows for 1,000 semantic keys. All-time H11a: 216,378 rows for 2,105 entity codes, 214,273 duplicate rows. entity_labels and registry_changelog are explicitly birth-exempt but retain birth triggers; system-wide there are 78 policy/trigger contradictions.

All live DB/config/service investigation was read-only. No cleanup, trigger disable, schema change, service stop, or patch occurred. After proof, five required report documents were uploaded to Agent Data.

The ~5,000-row spike did not continue during audit. Count 1,210,686 → 1,210,699 had 13 knowledge_documents; after five audit report uploads final count was 1,210,704. Faulty H11a remains scheduled every three hours.

Recommended fix: contain H11a via approved config/DOT; coalesce system issues; enforce birth policy in gateway; remove exempt triggers through governed migration; add paired monitoring.
