Had Phase C executed: patch_ops_code deploy writes a timestamped backup of
dot/bin/dot-apr-execute; rollback = re-apply the backup via an equivalent governed
patch_ops_code APR (never raw file write). Handler_ref binding rollback = governed reverse
migration. dot-c1-grant-issue rollback = DOT retire lifecycle. None needed now.
