Codex Review - Phase-1B Runtime Truth & Blocker Decision Packet
Codex Review - Phase-1B Runtime Truth & Blocker Decision Packet
Date: 2026-06-17 Agent: Codex Mode: independent adversarial control review Scope: read-only review of Phase-1B packet and execution report
Verdict
STATUS: PASS_WITH_MINOR_METADATA_TYPO
Phase-1B is a valid decision bridge. It converts the accepted Phase-1 runtime truth baseline into an Owner-facing blocker matrix, documentary-lag separation, corrected findings register, R1-R5 macro packages, execution order, TD gate, and OD-1..OD-8 decision prompts.
No Phase-1B blocker is falsely marked resolved. No remediation package is write-authorized. No Claude patch is required before Owner chooses the first package.
The only defect found is non-material: the execution report lists the decision packet content_length as ~25,800, while Agent Data metadata reports 32902. The packet body is present, rev1, untruncated, and internally complete; this is a metadata typo, not a material review failure.
3 Cau Tuyen Ngon
- Vinh vien: This review does not fix an incident or change runtime; it validates whether the decision bridge preserves the Phase-1 runtime truth and keeps future fixes Owner-gated.
- Nhamm duoc khong: The packet uses explicit non-authorization, per-row blocker status, forbidden actions per package, and OD prompts to prevent accidental remediation/TD/implementation.
- 100% tu dong: N/A for implementation; this mission is a control review only. For future work, automation remains gated by R1-R5 and the existing DOT/Owner approval discipline.

Primary Questions
-
Is Phase-1B a valid decision bridge? PASS. Packet section 0 states Phase-1 runtime truth -> Owner decision -> remediation macro workstreams -> later TD. Sections 4-9 implement that bridge.
-
Did it avoid remediation / technical design / implementation? PASS. It forbids live query, DDL/DML, runtime patch, source patch, current corpus, draft adoption, full TD, schema/table/index creation, materialization, DOT/KG/promote/birth/repair execution, migrations, and write-tests. R1-R5 are future Owner-gated packages only.
-
Did it correctly apply the two Phase-1 caveats? PASS. PH1-F1 is corrected to Dieu 39 / KG provenance, HIGH. PH1-F3 is split HIGH for birth-dependent TD and MEDIUM for non-birth scopes.
-
Does the blocker decision matrix correctly reflect Phase-1 runtime truth? PASS. The matrix matches Phase-1: RISK-BYPASS PARTIAL, HOLD-1 LIVE pilot-exercised, HOLD-2 PARTIAL, Dieu 39 REGISTERED_NOT_EXECUTED, Dieu 35 NOT_PRODUCTION_READY.
-
Are R1-R5 remediation packages correctly scoped and ordered? PASS. Default order R1 -> R2 -> R3 -> R4 -> R5 is defensible; R4 GUC confirmation and R1/R2 parallel scoping are noted as Owner-gated adjustments, not authorization.
-
Does it keep technical design gated? PASS. Section 6 keeps full TD gated and only permits scoped read-only/diagnostic work after Owner opens a package.
-
Is any blocker falsely marked resolved? PASS. The packet explicitly says every blocker remains OPEN and "No blocker is marked resolved."
-
Is any package over-authorized? PASS. Each package contains forbidden actions and write-enabled approval requirements. No package authorizes writes, DOT execution, materialization, schema changes, GUC writes, or gate flips.
-
Is a Claude patch needed before Owner chooses the first package? NO. The
~25,800content_length in the execution report is a stale approximate metadata typo. It does not alter the decision matrix, caveat application, package gates, or Owner decisions.
Caveat Audit
C-1 PH1-F1 surface correction: PASS. Evidence in packet: PH1-F1 row says corrected surface is Dieu 39 / KG provenance, HIGH, universal_edges=2199, 0 provenance, "no provenance=quarantine." It explicitly rejects the old RISK-BYPASS classification.
C-2 PH1-F3 split: PASS. Evidence in packet: PH1-F3 is HIGH for birth-dependent/canonical-dependent TD and MEDIUM for non-birth scopes.
Documentary-Lag Audit
PASS. Section 3 separates documentary lag from runtime blockers:
- RISK-BYPASS: old active bypass; runtime auto-approve neutered, residue remains.
- Dieu 39 owner: old GOV-KG-SYS unregistered; runtime registered active but inert.
- HOLD-1: old liveness unproven; runtime pilot lifecycle exercised, idle since 2026-05-27.
- HOLD-2: old no atomic promote; runtime fn_iu_enact atomic for IU lineage, birth/stamp path blocked.
It does not treat documentary lag as blocker resolution.
Matrix Audit
PASS. Required rows are present:
- Dieu 39 surface: HIGH, blocks Dieu 39 TD/implementation, R1.
- PH1-F1: HIGH, R1.
- HOLD-2 surface: PARTIAL/scoped, R2 + R5.
- PH1-F3: HIGH birth-dependent / MEDIUM non-birth, R2.
- Dieu 35 surface: HIGH/systemic, R3.
- PH1-F4/F5: MEDIUM, R3.
- RISK-BYPASS surface: PARTIAL, R4.
- PH1-F2/F7: MEDIUM, R4.
- HOLD-1: LIVE / MEDIUM, R5.
- CONS-002/003 + CELL + GOV blockers: carried prerequisite gates, not resolved.
Package Audit
PASS.
R1 forbids edge writes, provenance materialization, KG DOT execution, kg schema change, and TBox mutation. R2 forbids birth writes, certify execution, BIRTH_STAMP/PROMOTE_STAMP materialization, Dieu 0-G schema change, and atomic-promote build. R3 forbids metadata writes, DOT/health execution, fn_dot_enforce_paired creation, dot_config writes, and system_issues auto-fix. R4 forbids residue auto-revert, GUC writes, warn-to-block flip before Dieu 35 criteria, and approval-path code change without separate authorization. R5 forbids IU/staging writes, gate flips, promote/cut execution, and pilot-warning flips.
Owner Decision Audit
PASS. OD-1..OD-8 are decision prompts, not actions:
- OD-1 accept packet after review.
- OD-2 documentary refresh.
- OD-3 choose first package.
- OD-4 decide 170-row residue disposition.
- OD-5 confirm app.bypass_birth_gate and warn-to-block policy.
- OD-6 authorize R2 read-only diagnostic.
- OD-7 reconcile 1 undocumented admin fallback.
- OD-8 keep CONS/CELL prerequisite gate.
No OD is treated as already authorized.
Non-Authorization Audit
PASS. Phase-1B does not authorize or perform live DB/runtime query, DDL/DML, remediation, implementation, TD, current corpus creation, draft adoption, blocker closure, DOT/KG/promote/birth/repair execution, schema changes, materialization, GUC writes, or authority-order/baseline changes.
Step Report
- Step 0 foundation: read skill, OR, constitution/law anchors through Agent Data; applied no-background-agent rule.
- Step 1 receive mission: control review only; no runtime query, no implementation.
- Step 2 design before action: review method was source comparison: packet/report -> Phase-1 baseline -> law anchors -> checklist.
- Step 3 code: N/A, no code changes. Only this local report file was created to satisfy AGENTS report requirement.
- Step 4 two hats: Coder hat N/A; reviewer hat performed adversarial matrix/package/gate audit.
- Step 5 verify: production/runtime verification N/A by mission prohibition. Real evidence pasted above from Agent Data/local read outputs.
- Step 6 report: this file.
Final Decision
Owner may proceed to choose the first remediation macro package without requiring a Claude patch to Phase-1B. Recommended first package remains R1 by default, with R2 parallel scoping available if Owner explicitly authorizes it. No remediation, TD, implementation, or documentary refresh is authorized by this review.
